So they always expect things to turn out the worst they can imagine, yet they still try anyway.
yet as they are suck at level 4 or 5 they can't tell if they are easing or increasing those they are trying to help.

Chthonics know this, and so they don't even think about consider how their actions will make others feel.

Chthonics feel all the suffering they cause others 100X more than their victims, but are able to resist trying to help people anyway. Chthonics are stuck at level 4

The worst that can happen is what they were expecting anyway, so it is impossible to let a Chthonic down and since they are constantly being surprise by how much better than expected, they are always happy even when they are mopey.

This makes Chthonics appear unable to empathize with anyone else like a narcissist, in reality that feel the pain of others even more acutely than others
